Background

ACSA is a nonprofit, multi service agency, addressing needs and empowering children, youth, newcomers, homeless, and otherwise underserved communities.  Our core programs include housing and outreach programs, food security programs, newcomer services, youth outreach and therapy programs, child and family programs, early years’ programs, computer access programs, and community engagement.

 

Summary of Job Functions

This project is funded under the Home for Good (H4G) Layered Supports umbrella. The H4G Case Manager will provide intensive case management support to 10 individuals who have been assessed as being in need of a high level of support requiring frequent contact in order to sustain their housing.  Activities including connection with appropriate services, housing stabilization, eviction prevention and rehousing as necessary. The Case Manager will work within a housing-first, client-centred, and harm reduction approach, in a multi-disciplinary team approach to support clients with complex needs to maintain housing and to be linked with other support services, as determined by each individual client. 

 

Duties include:

  • Provide case management supports to 10 clients using a strengths-based framework to develop service planning and linkages, goal setting, and supports to stabilize and sustain tenancies.
  • Build and maintain responsive relationships with landlords to create positive tenancies, troubleshoot and support the resolution of tenancy issues, mediating when required.
  • Receive and follow up on referrals according to City of Toronto Coordinated Access and ACSA processes; follow all Home for Good Project requirements; initiate Case Transfer meetings.
  • Provide individualized and flexible case management services, through a housing-first approach.
  • Assist clients to identify and achieve goals in relation to housing, income supports, substance use, physical and mental health, food security and other service needs as defined by each client.
  • Assist clients towards integrating into the community, and empowering their use of resources and services to maximize their independence and potential.
  • Provide informed, managed referrals to other services including Rent Bank and legal services, Toronto Employment and Social Services offices, and other agencies.
  • Provide harm reduction counselling to identify strategies that reduce negative impacts of substance use on tenancies.
  • Accompany clients to appointments, including medical and Landlord and Tenant Board hearings as required.
  • Work with clients to complete community mapping, crisis plans, discharge planning.
  • Complete service linkages and referrals to ACSA’s income tax clinic, medical clinics, Drop-Ins, food bank, along with other service providers within case management team, ensuring easier access points to services.
  • Maintain accurate case management records, using Pirouette, and complete statistical and qualitative reporting as required by funder and supervisor.
  • Perform the responsibilities of the position within the legislative and regulatory standards set out in the applicable Acts. Performs the responsibilities of the position consistent with the policies of ACSA.
  • Protect own health and health of others by adopting safe work practices, reporting unsafe conditions immediately. Follows all guidelines for employees and employers as legislated under the Ontario Occupational Health and Safety Act.
  • Support the operation of the Homeless and Outreach Services Department as required, including Street Outreach, Food Bank, Drop-in and other services.
  • Other duties as required.

 

QUALIFICATIONS

  • A college diploma or a degree in human services, or equivalent of education and experience
  • Two years’ experience providing intensive case management services to clients with complex needs of people who have been homeless
  • Demonstrated understanding of housing, health, trauma, substance use, financial, legal, and other issues that impact clients’ lives.
  • Strong understanding of market rent and subsidized housing systems and policies, keeping up-to-date on emerging trends and changes; knowledge of the RTA.
  • Demonstrated understanding and commitment to harm reduction framework and housing-first approach.
  • Ability to work independently and as a member of a team, and to manage time well.
  • Ability to manage issues tactfully, and relationships with professionalism, and resolve conflicts.
  • Experience with maintaining statistics, writing program reports and developing and implementing evaluation tools.
  • Strong written and oral communication skills.
  • Crisis Intervention, First Aid and CPR certification.
  • Competency in Microsoft Word, Outlook and Excel, as well as data reporting programs.
  • Knowledge of community resources for homelessness, health, mental health and substance abuse.
  • Experience working with diverse communities.
  • Demonstrated engagement skills with a variety of stakeholders including residents, service providers, etc.
  • Strong organizational and time management skills.
  • Strong crisis management skills.
  • Experience working with partners.
  • Ability to work independently and part of a team.
  • Strong writing and communication skills.
  • Experience providing supports to homeless seniors is an asset
  • Fluency in a second language is an asset (Mandarin, Tamil).
  • Proficiency in Microsoft Words, Excel, and Power Point.
  • Valid driver’s license, clear driving abstract and a vehicle
  • Valid Police Record Check submitted within one month of contract signing.

 

Working Conditions

The Home for Good Case Manager works from various locations in the community, meeting with clients in housing units and in the clients’ buildings, public places and offices. The Case Manager also works from an office at 4155 Sheppard Avenue East and attends meetings and appointments anywhere in the City of Toronto as required.
